Grant Application Process: Guidelines

The following outlines the information required for a complete proposal, which should be between 4-8 pages, exclusive of budget and other requested attachments:

• Organization name, program name for which funding is being requested.
• Amount requested and the proposed one-year grant period.
• Contact person’s name, mailing address, phone, fax and e-mail.
• Executive summary, not to exceed one page.
• Brief overview of organization (if new to MZHF).
• Statement of specific health problem/needs assessment to which proposed program is responding.
• Proposed target population, estimated number/s to be served, and how the program fits within MZHF’s funding priorities.
• Program goals and strategy to achieve goals.
• Proposed impacts on the individual, family, organization, San Francisco community and/or Jewish community.
• Timetable/duration of the program, including when requested grant funds are needed.
• Partnerships/collaborations, if any, with other programs/agencies.
• Programmatic milestones/benchmarks and how progress will be monitored.
• Qualifications of current or proposed program staff.
• Plan for financial sustainability beyond the proposed grant period.

Please also attach the following:

• Total program budget, including line item breakdown of revenues and expenses for the proposed grant period, staff FTE commitments, and specifically noting the portion of the budget for which assistance is requested from MZHF.
• List of committed, pending and proposed funders for the proposed program, and the overall organization, including specific dollar amounts.
• Current year organization budget, including line item breakdown of revenues and expenses and staff FTE commitments (please specify fiscal or calendar year).
• IRS determination verifying 501(c)(3) status.
• List of Board of Directors.
